Creating & Sending a Form

Overview: Admin can create form templates and send forms to other users for completion. 

Create a Form

Learn how to create a custom form in Spry.

1. Begin in your Admin portal.
2. Navigate to “Paperwork
3. Select the blue “Templates” button.

4. Click the blue “New Template” button at the top right of the screen.

5. Next, select short or long form.

  • Short forms are those that you wish to create directly in the Spry app. These are customizable forms.
  • Long forms are PDF forms which you’d like to share digitally using the Spry app.

6. Enter the title of your form.
7. Provide a brief description of the form. 
8. Then, select self-report or mandatory-report.

  • A self-service form is a form that student-athletes can access and send on an ongoing basis. An example might be logging study hall hours, academic assistance request, etc. 
  • A mandatory report form is when the form is required for completion.

9. Next, you’ll be prompted to add questions to your form. Use this guide to learn about each question type that you can use when creating short forms. 
10. Once you’re ready, select the blue “Next” button at the top right of the page. 
11. Set the workflow for this form. 

  • For Mandatory Report Forms: Determine "Send Access". Choose will will have access to send this form.

  • For Self-Service Forms: Choose submitters and assign questions to establish the workflow.

12. Determine if this form needs an approver. Add the approver to the workflow.

Long Forms

Long forms allow admins to upload a PDF document and request signatures with Spry's DocuSign integration.

1. Click the blue “New Template” button at the top right of the screen.
2. Select "Long Form".
3. This will direct you to DocuSign. Note: First time users will need to create and verify their DocuSign account.
4. Set the roles. You can simply type the user who will need to complete the long form.
5. Next, upload your PDF form
6. Add your required fields.
7. Once your fields have been added, save the form. Once saved, you can return to Spry.

8. Your form is now ready to send. 

Send a Form

Learn how to send a form in Spry.

1. Navigate to the “Paperwork” page of your admin portal.
2. Select the "Templates" tab.
3. Select the form you wish to send. Click "Send". Note: You cannot send Self-Service Forms. Once created, these forms are accessible by the user who has "Send Access". For example, if you give "Coach" Send Access, a coach can access the form and submit it without the form being sent by an admin. Tip: Review this guide for more.

4. Select the users you wish to send to: Student-Athletes, Coaches, or Admin.
5. Then, select your recipients. To send a form you have the option to select your recipients by:

  • Individual
  • Team
  • Class
  • Tag

6. Once you've selected your recipients, click the blue “Next” button.
7. As an option, you can also share form visibility with other Admin or Coaches and set auto-reminders.
8. Once you send, recipients will receive a notification to quickly complete the form from their device.
