Overview: As a Coach, you can choose from a variety of elements and question types when creating a short form from scratch in SprySign. This guide discusses the available options.
Elements & Question Type Options
Learn the various question types that can be added to a SprySign short form.
Heading
This option can be used to section off different groups of questions throughout the form. This helps keep it organized and easy to read.
Statement
This option requires no action from the user. This should be used to share instructions or any message that doesn’t need a response. The user can also include a link within this option.
Divider
Inserts a line break in your form. Useful for separating sections and makes it easier to read.

Short Text
Simple, text-based question in which the respondent inputs a few words of text.
Long Text
Text-based question in which the respondent inputs text that is longer in length.
Dropdown
Allows the respondent to select single or multiple options. This option is best used when the user has more than five options.
Multiple Choice
Text-based question in which the respondent has various options to select. This option is recommended when the user has, at most, five options.
Scale Rating
Text-based question in which the respondent selects a rating on a scale.
Matrix
Allows the user to build a table to collect data from the respondent.
Legal
Allows the respondent to accept/not accept a statement.
Signature
Allows respondent to provide a digital signature directly from their mobile device.
Date
Allows respondent to select a date directly from a calendar.
Attachment
Allows the respondent to attach one or multiple files.
Insert Image
Allows the user to insert an image, such as a logo, directly into the form for viewing.
Website Link
Allows the respondent to paste a website link as the answer, formatted in http:// or https://
