*NEW* How to Create a Recruiting Questionnaire

Overview: Learn how to create a public questionnaire to publish on your program website, social media accounts, and more.

Our new Recruiting Questionnaire V2 was built to elevate your recruiting experience:

  • Enhanced Customization & Personalization: Make the questionnaire your own. You now have full capability to reorder questions, create sections, rename fields, and integrate your school’s unique branding, including your logo and team colors.
  • Advanced Data Mapping: You can now map specific questionnaire responses directly to designated areas of a PSA’s profile. This ensures all your critical data lives in one place, keeping your recruit profiles clean and organized.
  • Expanded Questionnaire Management: You now have the capability to build and manage up to 5 distinct questionnaires at once. This gives you the freedom to gather specific data from different segments of recruits without cramming all your questions into a single form.

This article covers the following:


Creating your Questionnaire

  1. Navigate to Questionnaire V2 under the Recruiting dropdown on your navigation bar.
  2. Select Add Questionnaire to begin building a new questionnaire.
  3. Enter a Name for your recruiting questionnaire and upload your school logo at the top of the Questionnaire.


    *Name, High School Grad Year, and Email Address will already be added to the questionnaire to get you started.
     
  4. Use the Form Design section on the right to change the background color and/or pattern of your questionnaire.


     
  5. Use the field panel on the left to drag and drop the types of questions or fields you need into the questionnaire builder.
  6. Map each question to the correct data field on the PSA Profile.

    To ensure responses automatically populate prospect profiles:
    • Choose the corresponding PSA profile field from the list of available fields.
    • Once mapped, responses submitted through the questionnaire will automatically update the associated PSA profile field.
       
  7. Configure each question's settings:

    Update the field label if you would like prospects to see a different question name or field label.
    • Add answer choices when using dropdown, multiple choice, or similar field types.
    • Enable or disable the Required option based on whether the prospect must complete the field before submitting.


       
  8. Create New Profile Fields (If Needed)

    If the information you want to collect does not already exist as a PSA profile field:
  • Drag and drop the appropriate element type into your questionnaire (i.e. short text, long text, dropdown, etc.)
  • Select Create a Custom Field
  • Enter a name for the new field
  • Click "Map Field"
  • Type the question how you would like it to appear on the questionnaire 
  • Make it required, if applicable


Rearranging and Formatting your Questionnaire

  1. Move the questions up or down within the builder to reorganize them in the order of your choice.
  2. Organize your form into logical sections using headings, dividers, or statements as needed.


Configuring Questionnaire Settings

  1. Navigate to the Settings tab at the top of the page
  2. Add a questionnaire description
  3. Customize your questionnaire's URL
  4. Add a Thank You message


Reviewing, Activating, and Sharing your Questionnaire

  1. Review Your Mapping
     
    • Click through each question
    • Verify that every question is mapped to the correct PSA profile field
    • Confirm field labels, answer options, and required settings are accurate
       
  2. Preview the Questionnaire
     
    • Select Preview Form
    • Verify the flow, wording, and required fields function as expected
       
  3. Save and Activate
     
    • Once satisfied with the questionnaire, click "Save"
    • Then, click "Actions" and "Activate" to make your questionnaire live
    • You are now ready to begin collecting prospect submissions!
       
  4. Share your Questionnaire using the URL and/or custom QR Code
