Create a form

Overview: This guide will teach you how to create a form in SprySign.

Create a Form in SprySign

When creating a form, you can choose to "Start from Scratch" "Import PDF" or "Use Template".

1. Click "SprySign" in your Admin Portal.

2. From your SprySign dashboard, click the blue "Create Form" button in the top right corner.

3. Next, choose an existing folder or create a new one. Form folder examples include: Compliance, Recruiting, Travel, Business, etc.

4. Next, choose how you'd like to create your form in SprySign. You can select from three options:

  • Start From Scratch: Create your own, custom form.
  • Import PDF: Import an existing PDF document and add the fillable fields.
  • Use Template: Select from Spry's template library and modify the form to meet your needs.

Create Form: Start From Scratch

Follow the helpful steps below to build a custom form in Spry. 

1. Choose "Start From Scratch" from the form template creation options.

2. First, add a name for your form.

3. Next, drag and drop from the left to add your form elements. Learn more about the form elements by reading this article.

4. After adding a form element, you can choose to make that field mandatory. After doing so, you will see a red asterisk (*) next to the field name.

5. Easily duplicate a question or element, by selecting the "Duplicate" button. This will copy the element and all question properties (listed under "Settings").

6. When finished, click the blue "Next" button in the top right corner to create the form.

Element Settings

1. After adding an element, click the "Settings" button to view more element settings.

2. View element settings and find additional options for your element.

Additional settings include:

  • Field Label: Change the name of the field
  • Pre-populate with System Data: This will autofill the question for any end user who is completing the form. For example, if this option is selected for "Full Name" a student-athlete's name will be autofilled when they complete the form. 
  • Add Tips: Use this tool to add helpful tips to your questions. This will add a small "i" icon next to the question. When hovering over the icon, users will be able to read information about how to complete the question.
  • Multiple Selections: In some cases, you may allow users to select multiple options. For example, you can enable the sport dropdown to allow multiple selections for multi-sport student-athletes. 

Conditional Logic

1. Select the "Conditions" tab at the top left of the screen.

2. Select "Add Condition".

3. Complete the required fields.

  • If: Select the conditional question or element.
  • State: is equal to, not equal to, is filled, is empty.
  • Value: Select the appropriate conditional question or element value.
  • Do: The action based on the "If" question 
  • Field: The questions or elements to which this logic is applied. Here, you can select multiple.

4. Save your condition when completed.

Create Form: Import PDF

Upload a PDF to create a form in Spry.

1. First, drag and drop your form OR click the blue "Upload Document" to select a file.Screenshot 2024-07-18 at 6.34.01 PM.png

2. After importing your form, drag and drop from the left to add your form elements. Learn more about the form elements by reading this article.

3. Resize the element fields as you need. 

4. After adding a form element, you can choose to make that field mandatory. Click on the element and select "Required". After doing so, you will see an asterisk (*) next to the field name.

5. When finished, click the blue "Next" button in the top right corner to create the form.

PDF Element Settings

1. After adding an element, click on the element to view the element settings.

Additional settings include:

  • Field Label: Change the name of the field

Create Form: Use Template

Select from Spry's Template Library.

1. After selecting the "Use Template" option, choose a template that you'd like to add to your account.

2. Search or select templates in the Spry library. 

3. After selecting a template, you will then be able to modify it to meet your needs.
